Health & Safety Enrollment Policies

Registration and Payment You must pre-enroll by downloading the course registration form (PDF version) and submitting (via email health&safety@nwarkansasredcross.org or Fax 479-306-4325 or mail to NWA Chapter PO BOX 789, Tontitown, AR 72770 ATTN: Health & Safety) up to two business days in advance of the course. Class fees are due at the time of enrollment. No space is reserved without advanced payment. Payment may be made by cash, check or credit card (Visa or Mastercard or Discover or AMX). Please include your 3 digit CVC2 code located on the back of your credit card. Those who pay by check must note the name and date of the course in the Memo line. Please include the name of the credit card holder if it is not the under the name of the individual who is registering for the course. Companies and organizations that pre-enroll employees may be invoiced if requested and will be billed for all pre-enrolled employees in which a space is reserved.
Cancellation Policy We require two business days notification to cancel your enrollment and receive a refund or transfer enrollment to another class. A $10 administrative fee is charged if enrollment is cancelled. If you do not attend the class in which you enrolled, you will not be issued a refund for the payment of the class. Worksite Training locations will be billed for the total of pre-enrolled employees in which a space is reserved. A minimun of eiqht registrants is required for all worksite training.

Northwest Arkansas Chapter, American Red Cross 250 Industrial Circle East Tontitown, AR just past 112/412 Intersection) (479) 306-4688	Worksite Training brings American Red Cross Health & Safety courses to your location at your convenience. Red Cross instructors conduct the same quality Health & Safety courses for eight or more people, 24 hours a day, seven days a week at your place of business. Due to service delivery requirements, your business will be charged for the number of registrants prepared for, unless notified prior to the class. Min. requirement for a full-service class is at least eight people. Please call or fax at least two weeks in advance to reserve your date. To schedule worksite training (Full Service), please call (479) 306-4688 and ask for Health & Safety Services or complete Full Service form and fax to 479-306-4325 or email to health&safety@nwarkansasredcross.org . You will be contacted to confirm training schedule.
